The first patient "grandpa" biden is suffering from the delusion that he is running the world and has the fate of the world in his hands.
To help with his care, the facility has provided a large globe so he can survey his "dominions" and a big red button labelled "the bomb".
Also, members of the staff dress up and pretend to be journalists holding a press conference to keep grandpa happy.
Lookalike world leaders also dress up and come and visit grandpa for which they are paid handsomely from Grandpa's many friends and associates.
